Jack Leonard Crouse, Jr., 47, formerly of Madison passed away in Massillon, Ohio April 5, 2016.
He was born June 9, 1968, in Geneva, the son of Jack L. and Donna Joan (Davis) Crouse. Jack graduated from Madison High School, and went on to receive his bachelor's degree in Education from The University of Akron.
Jack was an avid sports fan, particularly enjoying the sport of baseball, which he enjoyed playing during his youth. Jack also enjoyed working on and learning about computers and technology.
He is survived by his mother, Donna Joan Crouse of Madison; sisters, Pamala Ensell of Madison, Terri (Cory) Flensburg of Concord, and Cynthia (Brian) Sullivan of North Olmsted; as well as many nieces and nephews.
Jack was preceded in death by his father Jack Leonard Crouse, Sr.; and his grandparents.
A memorial service for close family and friends will be held at 11:00 AM, Wednesday, April 20, 2016 at The Behm Family Funeral Home, 26 River St., Madison, Ohio 44057. Friends may call from 10AM until the time of service. His final resting place will be at Madison Memorial Cemetery on Arcola Rd.
